polldate라는 필드가 어떻게 되어있나요??
위에러(out-of-range) 는 데이터 형식이 맞지 않아서 나오는 에러입니다..(범위를 벗어났다는...)
polldate라는 필드를 smalldatetime형식을 줘보세요..
아니면.. date()로 input하지 말고.. getdate()로 insert해보세요..
아니면 날짜를 now()로 받아서 insert해보시던가..
